Glenn Procedure BDCPA

This video shows the surgical technique for The Glenn Procedure. This operation is also known as the BDCPA, or Bidirectional Cavopulmonary. Several other names are used as well. This operation serves as the second Stage Palliation for Hypoplastic Left Heart Syndrome (HLHS), and other single ventricle hearts. A common strategy for newborn single ventricle babies is for them to undergo an initial palliation in the first week of life, often a shunt, sometimes a stent in the patent ductus, followed in about four months by the Glenn procedure. In this procedure, the surgeon replaces the shunt by detaching the superior vena cava from the heart and connecting it directly to the pulmonary artery. This operation then paves the way for the third and final Palliative operation, the Fontan, where the inferior vena cava is detached from the heart and connected directly to the pulmonary arteries. The Fontan is usually performed at around 4 years. (A useful way to remember the timing -- first stage at 4 days, second stage at 4 months, third stage at 4 years and the single ventricle palliation is complete), this of course may vary between individual babies and heart programs. The video is graphic, showing the actual operative technique, and the patient's day by day postoperative recovery. Results for this operation will vary significantly between institutions.
