Ce morceau est le 4e single (le 5e si l'on inclut This Time Around) de l'album aux États-Unis et le 4e en Europe. En Angleterre, "They Don't Care About Us" atteint la 4e place et reste dans le Top 100 pendant 18 semaines, alors qu'en Italie et Allemagne il est numéro un. Ces résultats contrastent avec le Billboard Hot 100 où le single ne dépassera pas la 30e position même s'il y restera 13 semaines.
